Eligible Employees: Employees scheduled to work 30 hours or more per week are considered full-time. All bi-weekly and weekly paid employees are eligible for benefits on the first day of the month following 60 continuous days of continuous service. Eligible Dependents*: Your legal spouse or domestic partner Your children up to age 26 (including biological children, legally adopted children, stepchildren, foster children, and children for whom you have been court appointed legal guardian) Your children over age 26 who are dependent on you for financial support and are not able to support and care for themselves due to a physical or mental disability If adding a domestic partner or child(ren) of a domestic partner to your medical, dental, and/or vision plan, it is important to understand that your portion of coverage will be deducted from your paycheck as pre-tax, and your domestic partner and domestic partner’s children’s coverage is deducted as post-tax. This will be reflected on your paycheck as “imputed income”. A Domestic Partner Affidavit is also required before coverage begins. *NOTE: All dependents must be verified before coverage can begin.
